1. Has the noise progressed as the Millage increased?
2. Has anyone taken them off road and has an update?
3. Any updated comments

I've had mine for over 30k miles now. My opinion on them has dramatically changed. Most seem to love these tire, but I hate them.
1. After 20k, noise has gotten much worse. They're almost as loud as the Toyo MT's my uncle runs (only tire I have to compare to). It's primarily at 30-50 mph. I don't mind the noise, but for the amount these tires make I might as well be driving on something aggressive that should make this much noise.
2. I don't "off road" in the typical sense of the term. I use my 4runner to get to/from my hunting spots, which does involve a lot of off pavement driving. They've not let me down here, overall very pleased for an AT tire. One thing I will mention is that they don't pick up rocks as bad as other tires, that's a big plus. I've not really pushed them to the limits, but they've always got me home.
3. My main issue is performance in wet conditions. After 20k miles, they are horrid. Driving even in light rain feels like your driving in snow. Even though the 4runner is full time 4wd, I've caught it slipping and sliding on numerous occasions. I've almost wrecked a couple times because under mild to moderate braking they lost traction and I slid for quite a ways. I had a very close call a week or two ago, and that drew the line for me. After a little over 30k miles, I'm replacing mine with something more reliable. I'm not taking a chance on my families safety. I can't imagine how these would handle in severe weather such as snow and ice. I can honestly say I'd drive my wife's Camry over my 4runner if there was snow on the ground (luckily we don't get much here). I do not drive crazy. During normal driving, these tires have really let me down.
Wear has been good so far. If it wasn't for the issues mentioned above, I could easily get 50k out of these. I do a five tire rotation every 7500 miles, and they've held up great. In 30k miles, I've had one puncture that was caused by a nail, so I'd say they do pretty well here as well.
Up to roughly 20k miles, I absolutely loved them. Performance all around was excellent. They did great in all conditions, and were much cheaper than "comparable" tires in other brands. I've ran Hankook tires for several years now on both mine and my wife's vehicles. Always been very pleased with them. I don't have much else to compare these tires to, but they have definitely fell short of my expectations.
With all that, I'll add a disclaimer. The noise is an issue no doubt. They are easily 2-3 times (if not more) louder than they were new. The wet performance could potentially be related to the size I'm running (255/75/17). The narrower tire does better at preventing hydroplaning (no real complaints here). However, it is possible that a wider tire would have better performance on wet surfaces (i.e. the road after a rain, no standing water). So, your results may vary.
The biggest issue I see is that when people write reviews online at the tire places, it's typically done as first impressions. I think a lot of what you read is a result of this. A lot of those people may still be pleased after putting some more miles on them, I'm just not one of them. I expect performance to suffer as tread wears, but not like this. That's my opinion, take it for what it's worth.
