It's an aluminum spacer that will give you 1.5 inches of lift. Basically, it's a leveling lift that will give you a bit more than the typical 1 inch leveling lifts that many people have installed, including myself.
 
Ok, so this goes on the rear and fixes the sag, is this for 2003 and up or earlier, and being a newbie, how hard is this to do?
 
No, it goes on the front and fixes the low nose to look more even with the rear. It's for '03 & up 4Runners. It's relatively simple to install.

I don't have a pic of this exact kit installed, but do a search on the forums and you'll see tons of 4Runners with different sized leveling lifts.
