There's always a risk, but mine were fine at ~270k miles - just don't lift the truck before replacing the ball joints. That's when they start to fail more often, especially when old. OEM only on those lower ball joints (I replaced mine when doing other suspension work, pre-lift). The upper ball joint is likely just fine (and is much less likely to fail catastrophically).
