First post for me and saw this thread describing a similar situation I'm having with my 2000 4Runner. Everything began with a bad #4 injector. So I replaced all 6 injectors, cleaned TB, IAC, MAF, new intake gaskets, plugs, wires, upstream O2 sensor. Truck ran great for a few hours but on the way home it started to idle rough and stall at a stop. Seems to improve once you get out of the gate but not completely. Here is a look at codes:

I'm having the same problem as you guys were having. What was the fix for you?
I have a 2000 with 215K. Just had all new gaskets, O2 sensors, injectors installed, and TB, IAC, and MAF cleaned. Fairly new plugs, wires, and coils. Still running rich. Stalled a few times at stop lights. Strong gas smell. Poor gas mileage. Code P0172.

Anyone ever figure this out? I had a misfire in cylinder 2, replaced coils, wires and plugs, misfire didn't go away so replaced fuel injectors with remans from Rock Auto. Everything was fine for like a day then it started running like crap and blew white smoke that smelled like gas from the tailpipe.
Seemed like it must be a leaky injector so I put one of the old ones back in on the cylinder that had a gas soaked spark plug (all the spark plugs were a bit black but this one was wet with gas) Drove it like 10 miles, then let it sit for like 45 minutes (this was always when the misfire would happen before) then drove it another few miles, everything seemed fine, no misfire, no white smoke, no cel.
This morning my wife went to drive it to work and it was running like crap and died at the end of our street and now it's throwing p0172 again.
Any ideas?

P0172 , + stalling at the lights...any other idea?
You may have a clogged cat. Especially if you drove it with a missfire. And even worse if you drove it with a flashing check engine light.

Could be a clogged cat I suppose, we never drove it more than a minute or 2 with a misfire or flashing light, it was dumping a lot of unburned fuel when I was getting the smoke and everything but I never ran it for more than like 2 minutes like that, had it towed home and everything. I haven't gotten a p0420 and it doesn't feel at all like my other car did when the cat was clogged. Theres no smoke now. I just cleaned the maf and I'll see if that helps in a bit.
Anyone have any other ideas? Hopefully cheaper/easier than a new cat?

Pull the vent line off the fuel pressure regulator and see if it’s wet.

Assuming the vent line is the small one that comes off the top it is not wet

I cleaned the maf with maf cleaner and cleared the code, after running for a couple minutes it's blowing white smoke again and smells like gas again, also running rough. Took a while to start too, cranked for a while a couple times before it actually started.

I'm thinking another injector is stuck open or maybe the maf or upstream o2 is bad but I can't imagine them causing it to dump enough gas to cause white smoke. It's p0172 so bank 1 so could I just put the old injectors back in on that side (and hope I don't grab the one that was clogged before, unfortunately I didn't mark them) or could too much fuel on either side cause that code?
Any other ideas on what it might be?
I did break a couple bolts on the last reinstall, even though I was using a torque wrench set to 13 ft lbs, maybe its way off somehow, but the last time it did this none if the bolts were broken or anything and the 2 that broke seem like they're not totally vital, although I obviously need to get new ones, one on the upper plenum and one on the throttle body, they still seem to be sealed and I'd get a lean code if they were letting air in right?

Here is my experience that I had recently with the P0172 code:
My truck started to run rough and actually died when I pulled into a parking lot. It seemed down on power, idled rough especially on a cold start up and just felt sluggish. My check engine light never came on but when I scanned it, I had a pending P0172 code.
Fast forward to what I replaced/fixed:
I replaced the upstream O2 sensor with a new Denso unit. I had to drop the CAT because both nuts on the O2 sensor were seized on and that's when I found a crack in the pipe, outside of the O2 sensor flange.
I welded the crack shut and installed the new O2 sensor. It's been running great ever since and I've picked up a couple MPGs. This was a few months ago so the problem was definitely fixed (no more code either).
Unfortunately, I don't know for sure if it was a bad O2 sensor or if it was from the crack since I fixed both at the same time.
