I would check for a vacuum leaks next. If you don't have a vacuum leak, I would try disconnecting the battery for 20 minutes and reset the computer.
Someone else may have some other suggestions.

Have you replaced the IAC? My runner had a stupid high idle when I first got it, I replaced the IAC, cleaned the throttle body, and replaced the MAF and the idle went down.

Cleaning the throttle body on the truck is a good way to flush gunk down into the IAC. Not saying you did that, just FYI.

Take the throttle body off the vehicle to access the IAC (including the coolant lines). It's held on by four phillips screws. Once open, you see the "barrel" looking valve at one end, that's what you want to clean out. Use MAF cleaner so it evaporates quickly and spray the heck out of it. Gently rotate it, GENTLY to make sure it's not sticking. You might also want to pick up a new IAC gasket from a parts store while you are at it to make sure it doesn't leak when you put it back together, since the gasket is all that separates your coolant from your air intake.
Not a hard job but can save you a pretty penny. Brand new an IAC valve costs $200+.

Only other thing I could think of is the that throttle valve or "butterfly valve" in the throttle body is warped and not shutting the throttle as much as it should. Very rare but has happened before.

Drop a TINY amount of general purpose oil in the hinges on both sides (engine oil will work in a pinch too). You might have to take the throttle body off again, since you have to apply it on the inside of the throttle body. Work the oil in by opening and closing the valve, might need a second drop of each side. WD40 is not the same, it does not have good lubrication properties.
Give that a try and see if it helps. I had to do the same on my old Toyota once.

Isn't the intake air temp sensor the one plugged into the side of your air filter box? Pretty sure that is where it is on the 3rz at least. I assume the v6 has one there too.

I have a 98 v6 auto 4Runner. It got to where when I would start it it would start right up every time and almost immediately die. Would start right back up and die again. Ended up replacing the iac. Now will start up and idle fine. Runs fine. But the issue I am having is lair example when I come to a stop sign it acts as if it shifts itself into neutral. I go to pull out it just revs up and when when the Rome start to come down it's like it shifts into drive. Seems to be really bad when shifting into reverse. Will eventually go into gear but I'm afraid this will eventually ruin my transmission. Never done this till I replaced the iac. Could the iac be bad maybe and causing this. Like I said runs fine just when I come to a stop will it do this. Thanks for any input.
