Foam Cover for PCV Hose Near Throttlebody

Is this foam cover still available? Happen to have the part number?

I know it certainly isn't critical for vehicle function(although I swear its why my 4wd won't engage sometimes ) but I wouldn't mind replacing it if I can. My PCV valve and hose were long overdue.
