Sounds like you've got some advice so far and it's pretty consistent with my experience. I've put a lot of time and research (and $'s) into adding more performance to the otherwise lethargic 5VZ-FE.

Intake - The most cost effective power adders are indeed removing the stock elbow or doing the deckplate mod. Is this useable power? No not really, unless you floor your 4Runner on a regular basis. But the gains are there at around 9 HP. Gadget, the owner of URD, has some data to back it up: Air Induction. If you'd like to do it though, you remove the air box (3 bolts) and if needed, the intake tube (2 clamps). You'll see 2 10mm bolts holding the intake elbow to the fender. Remove these and with a little wiggling, it will pop out into the engine bay. Tada, 9 HP gained.

Exhaust - Someone else hit this nail on the head. The downpipe and catalytic converters are 2.25" piping, smaller than the cat-back exhaust piping. Anything done downstream of the cat does basically nothing. I replaced the muffler on my supercharged engine with a Borla straight through and it did, nothing. Exhaust changes would be for sound only. Headers add power but it's not cost effective at all. For reference, on my highly modified engine I added +30 HP with headers and exhaust. That cost me around $2300.

I would not wish a header install on a 96-98 4Runner even on my worst enemy. I had a shop install it that I trust and it took him 2 solid days. He was a standup guy and only charged me 7 hours labor. The amount of welding and relocations needed is insane.

I still regret not putting headers on my 97 when I put the new engine in it.

Now that I know a company makes repair parts for the frame I’m committed to wearing out this new engine.

California emission vehicles with 2 cats are bolt on to the exhaust and the transmission dipstick doesn't have to be moved at all. The installer had to create a new upper dipstick tube and bracket, it pops out in the "loop" in the passenger's side header. 01-02's also have less clutter in the engine bay as things were relocated and went electronic.
