You mean factory installed wide flares right? There's plenty of 15" wheel 3rd gens that got the black plastic flares installed at port.

I'd be a bit surprised by that. The rear fender lips don't have the holes on the 15" trucks to mount the flares (I had to drill mine to add them). The front skinny flares bolt up just fine.
The door sticker almost always says 15" wheels, even on the 4R's that had 16" wheels - so don't let that confuse the issue.

Even the 16" wheel 4Runners only had little chrome strips around the fender lip unless they were optioned with flares or were a Limited (which always got 16" wheels, along with the cladding).

And yet here I am, currently sitting in a 4R that has fender flares and 4WD and originally was fitted with 15in wheels...

Hopefully that's not based on the door sticker... Because unless it also had the small brakes (front discs/calipers and rear wheel cylinders) and 3.7 or 3.9 gears, it was a 16" wheel truck.

Yup, that's accurate. I can look for it but the skinny flares came with instructions on where to drill into the rear fender.

It’s crazy that it only had 28,000 miles and still had some rust and oxidation. That’s probably why it only went for $25k being from NY most of its life.
