Per the requirements under the extended warranty, I took my T4R to a Toyota dealer during the time frame of the extended warranty, they got my VIN number, inspected my cracked dash and they determined that my T4R indeed was covered and the dash met the requirement for replacement, they then placed the order for my new dash and then when it came in they contacted me to make a appointment to install it, I took it in and they had it replaced in around 2 hours, it was 3 months in between the inspection and them installing the dash which was very good, many had to wait up to 18 months after their inspection because in the beginning Toyota was very back logged since they had to setup and make all the new dashes, I got in on the warranty in the last 6 months of it.
Unfortunately for those that didn't get their's replaced within the time frame, the deadline has long since expired to get Toyota to replace their dashes under the warranty, I feel for those who wasn't aware of the warranty and missed out on getting their's replaced, for those that did know and procrastinated on taking their T4Rs in for the inspection before the deadline so they would get on the list for replacement I can't feel bad for them, because as they say "you snooze you lose".
