2004
(Will look it up and edit with info)
4X4
204,000
Number 4
Edit:
For what it's worth, 200k miles is about when the problem started. Haven't replaced them yet. Was quoted $3,000 to fix it, so am considering selling it. I'm now at 218,000 miles and it's still running. My mechanic told me to try a chemical head sealer before I buy a new car or drop thousands of dollars on my 4Runner. Didn't think it would work. I'm not holding my breath, and, knock on wood, but Bar's HG-1 seems to have elevated the symptoms of a blown head gasket in my truck, so I'm hopeful it may have actually worked.

Just saw the thread, here are my issues:
2004 V6
HG leak at @125,000
Same problems as other posters, rough start and misfire on cylinder 6. Coolant also fouled oxygen sensor. Total bill was around $1900 to replace the gasket, check the heads and replace the o2 sensor.
Other issues:
Leaking crank shaft seal due to improperly machined harmonic dampner. Took forever to trace and find the source of the leak. While replacing the seal I also found that the radiator was leaking. This was around 130000.
No other issues since.

I called 800-331-4331 and sure enough no recall or anything, they just took my name down.
Here is my case# 1408152385,
Anyone who has had this issue even if it was years ago please call and mention my case number so we can get the wheels moving on a possible recall.
The rep did say if enough people called in that people who did the repairs previously could possibly be reimbursed. Please call.

my neighbor has same model 2003 sr5 v6
regular oil (mobile10w30 extended performance) every 3 k
no water pump changed coolant flushed at 100k with toyota oem
220k miles and still running strong
my brother has 2004 v6 sr5 regular oil change 10w30 extended p
every 3k
180k miles still running strong, never had water pump replaced, simply
oil change and coolant flush at 100k.air filter changed at each oil change

So far not 1 03' is on the list. I wonder if something changed between 03' and 04'? Mine is an 04' but built and purchased in 03', all the others in the list (from info given) are 04's and 05's built in 04' and later. Interesting and I will keep updating the spreadsheet when I get people to post new info.
I am really curious about the build date which many are not posting.

Too small of a sample to make any statistical conclusions.

Hey Mr. Statistician;
I am only putting together some basic info from what people are posting. I am no where near trying to make a "statistical conclusion".

Hey Mr JumpToConclusion,
Never said you were. I was just pointing out that you CAN'T make any analysis about variances from one year to the next from this small sample. I think your stats is good and useful, just NOT when trying to make conclusions.

Good Thread. Toyota has the data. If I was in your position I would request it. When they say no, have an attorney write them. That will get the ball rolling.

2004
11/03 production (IIRC)
4x4
150,000 miles
Cylinder 2
-Never overheated, coolant changed on schedule with Toyota SLL
**Fun fact: A head gasket leak is the reason I sold my '99 Tacoma 3.4 before this. Bought the 4Runner thinking I'd have at least a good 3 or 4 years before anything serious went wrong. Had it 9 months and this happened... Yay me.
