Long story short, if you have basic mechanical aptitude, a decent collection of tools or friends tools, and a place to do the work, I would encourage you to tackle this job yourself. Yes it seems daunting, but there is plenty of information out there to get this job done well and save yourself $2-$3k+. I was hesitant myself until I found a step-by-step video on performing the whole job on the exact same engine. Combine that with a Toyota service manual and performing your due diligence to prepare for the job, and it's really not too bad - just time consuming. Nothing wrong with taking your time, you don't want to open up the engine more than once.

Money is tight right now for my wife and I, so it was definitely worth it for us, but paying for the fix may be best for others. It can be done for ~$500 for parts and supplies needed.

I'm optimistic that there are many more years of life left in her.

Replacing the left bank HG, so technically both gaskets were blown.

I was aware there was a slow oil leak coming from that side of the engine for a while. It was slow enough I never really took the time to diagnose it. I was surprised when I took the left head off to find the oil leaking from the outside of cylinder one. Inspection of the heads determined the flatness was in spec so after cleaning up all the sealing surfaces really good I buttoned everything back up with all new gaskets from an engine gasket kit. She's been running great since.

Killed two birds with one stone with the fix. I'm around 2k miles with the new HG and no leaks so far

After 40k miles of thinking it was one thing or another, my symptoms got worse (misfire startups again, horrible fuel economy/reduced power, waterfall noise, coolant being pushed out of the reservoir). I finally took it in to a shop and had the headgasket replaced. They confirmed that it was leaking combustion out and coolant in on cylinders 4 and 6.

'04 V6 - 201k miles (likely been an issue since 159k miles when I bought it)

They also replaced timing components and a quite worn oil pump while they were in there. I updated the headgasket to the '05+ version to hopefully avoid future issues.

Has anyone yet done a write up of doing the head gasket replacement job, with pictures and part numbers?

The FSM is always helpful but I've had greater success with some sort of guide with pics that cover things not laid out clearly in the FSM.

There's a video YouTube that's literally a step by step on how to do it. But you will still need the FSM something for torque specs or anything else the might of skipped over in the video.
