I was hesitant like you, to do the atf exchange. When I did both my t4r and gx470 I ended up using Bout 14 quarts each. So I would recommend using more just to be sure you get all the old stuff out. It's really not that bad. Just go ahead and do it. And go ahead and do the filter change to. Because if you are going mess with it you are going to want to clean out the pan and you might as well change the filter while you are at it.

I suppose I shouldn't use the term "flush" so loosely since apparently that has the connotation of forcing a higher pressure through. I'm planning on doing a "fluid exchange" through the cooler lines, after draining the pan and refilling it, then once it seems clean coming out I'll try to set the level.

I get different thoughts on the filter replacement, some saying yes, some saying it's not really necessary. I've also seen people with the 2004 model saying they can't drop the pan without disassembling exhaust crossover, and others saying it's possible to do without disassembly. So I'm gonna get the 4Runner over a pit and see if it definitely looks like I would need to drop the pan.

Were you able to drop the pan without removing the exhaust crossover afterall? im considering the job but exhaust is so rusty that id rather not mess with it. Thanks.

I was not able to drop the pan without removing part of the exhaust, unfortunately.
