Replacing the Fuel Inlet Hose

Have had an issue with my CEL coming on with a P0455 code so I replaced the gas cap with a generic version, but the CEL came back on a few weeks later, so I then replaced with a OEM cap this time. This weekend however, I went to fill up and noticed a gasoline smell, looked under the rear and noticed a giant chunk taken out of the fuel inlet hose:



Anyone know of the part number (2004 V6 4.0L)? I'll probably call around later today but thought I'd ask the forum first. Seems like it would be an 'easy' fix

Thought I would update this post in case anyone else needs to make this fix in the future. I ended up ordering a new fuel inlet hose from Village Toyota (first link) with the exact partnumber @lightfreak posted (thanks!). Part came in quickly and I replaced it yesterday, it was a very easy fix.

Parts side by side, perfect match...




I'm still amazed at the size of the hole in this thing, not sure how the original owner drove around on it. You could only smell the gas at fill up, so I'm sure he filled it up prior to my test-driving it (sneaky b**tard ). I also can not figure out how the hole was made given it's location and no other visible damage to the surrounding area....

When reinstalling, put the fuel tank side on first (very snug) then attach the gas fill side, tighten the straps and you're done:
