2006 4Runner V8 has codes P0011, P0016, P0018 and P0021.

250 000km, timing belt (w/ tensioner and pump) done at 150k km.
Idle is very low and rough
Poor performance.

I'm thinking skipped timing belt tooth, any other ideas/theory/experience?

All of the codes listed seems to point to camshaft sensor A sensing the timing is off....BUT it also seems it's getting closer to the time for the timing belt to be replaced again.

It could also be a issue with the cam sprockets not functioning correctly which could be from blocked oil passages or wear, with the VVT-I engines the sprockets are how the system advances or retards the timing as the sensors senses where the timing needs to be, it could also be a bad sensor.

That being said if when the timing belt was done if they installed an after market belt instead of an OEM belt it's possible that the lesser quality belt is going bad and indeed skipped a tooth, or even if an OEM belt was installed if the water pump wasn't replaced it could have started leaking or the replaced one has started leaking and coolant will lead to premature failure of the timing belt.

I would suggest replacing the timing belt ASAP because if it breaks the pistons will hit the valves and the engine will be severely damaged and will need to be either rebuilt or replaced, also suggest replacing the water pump, checking the sensors and sprockets, etc.

For the most part much of the above is regular maintenance, so it sounds like it's time to bite the bullet and get it done.

I would start with removing the timing belt cover too get a look and see if the timing marks are off, inspect the condition of the belt, tensioner, etc. and go from there.

Getting to the belt cover to take a look seems like a big job by itself, might as well replace everything.
