I recall reading here on 4Runner forums that caster gain with the LCA rotates the spindle such that the lower ball joint mount moves forward, making the wheel further away from the firewall (where we normally have interference with the body mount).

The UCA does the opposite, moving the tire rearward, closer to the trouble area.

I'd have to assume that due to the geometry, 0.4 degrees was all that can be attained without affecting other aspects of the suspension.

Makes no sense to me to use the LCA for caster as it is "cocked" in position then and seems odd to operate it "cocked". I installed a cam eliminator kit and run it in the center position front/rear on LCA. Have an SPC UCA that I can adjust for caster/camber. I run stock sized tires and wheels and don't off road so probably a little unique in that respect.

I'm pretty set on getting a pair of the offset arms from SuperPro at this point, I am just nervous about install because their instructions seem very vague. How difficult was the install and how would I ensure my cams are close enough to get it to an alignment shop?

Also, once at the alignment shop (even if I take it to an offroad/high performance specialty shop), will they know how to use the adjustability of the arms that the bushing provides?

Maybe I'm not thinking about it right. If anyone could quell my nerves that would be sweet.

Measure your toe and camber BEFORE you install the SP LCA, it's easy to do.
Mark your LCA cam positions with paint pen or whatever. Install the SP LCAs and set your cams back where they were. Check your toe and camber, should be close to what they were. How much caster is SP LCA supposed to provide with no other changes? Don't worry about your caster and take it for a ride. how does it feel, does steering wheel return to straight after a turn with little effort, does it wander on highway or feel as it did before? If satisfied, don't worry about caster, IMO.
I installed SPC UCAs and have never taken to have it aligned. I checked and adjusted my toe and camber, had a two year old alignment sheet that told me what caster was and set the SPC one position positive, oh, and I installed a cam delete kit in LCAs and set all at center position, drives very well.

Which orientation is the bushing supposed to be in, given that its asymmetrical or "offset" as they call it? It seems to me like it either pushes the LCA out towards the wheel or pulls it in towards the truck, depending on which direction it is "offset" to. Does it come already in the configuration that you'd want for a lifted application (2.5" or so)? This is really the only thing left that I am unsure of.

This is how they should come, out of the box. These were mine when I did the install. Be sure to remove the blue cup from the balljoint before installing. It's only a protective cover for shipping.

Not the blue bushings themselves. Per the included (should have been at least) instructions, use anti-seize on the bolts that go thru the sleeves. You can and might need to use some grease (bearing grease or something of the like) on the outside of the bushings, just to help the LCAs slide into the buckets.

That install was brutal on my 16 year old New England truck. Took all of Saturday, Sunday, and Monday. Couldn't resist a quick joyride with all my new hardware despite not being professionally aligned yet. I have to say, these things are the real deal! Quiet, responsive, and tight. Alignment appointment today so we will see what numbers I can get!

Thanks everyone for the help. Finally some suspension mods to tout!

Looking forward to seeing what you came up with. About to pull the trigger on these. Not sure if I want to go with the offset ones or regular. I think I’m leaning towards regular because I haven’t had issues getting the alignment I want (usually shoot for 3.5-3.7 caster with jba ucas) worried if I get the offsets then I’ll have too much caster?

I was also worried about having too much caster. I pulled the trigger on the double offset ones and couldn't be happier. I also have JBA uppers, love those too. Likely you'd be able to get the same numbers with the standard LCAs but I don't know for sure. I do know that the SuperPro lowers give you a lot more adjustment room because you're not "maxed out" for lack of a better term.

Here's my alignment sheet, I asked for "between 3 and 4 degrees of caster":
