Grinding Metal Sound - Help Identify Issue (video included)

I've searched around and found several threads discussing sounds that may be the CV Joint or dust shield. I haven't found one with the exact sound recorded so I recorded mine hoping to get some feedback. It starts around .22 seconds and last about 5 seconds. Sometimes it is when I crank it, sometimes when putting into gear, and sometimes when I press the brakes (but doesn't stop when I let go of the brakes). Hopefully this is good enough quality to hear it. Really random.

2003 4wd V8 (198,000 miles). Just put on new brakes up front thinking that was it but it wasn't. I do have a busted sway bar that I need to replace. Just noticed this morning that when in 4lo my truck won't shift into 4th gear. Not sure that is related but I thought I would share. I checked transmission fluid. It looks red as it should but I did notice what appears to be a little rust when I wiped off the dip stick.

New Video with better sound: YouTube
