Lots of people have, here are some answers from someone that has a JBL stock system.
1. Yes, there is a flat portion on the left side of the enclosure that holds the normal stock JBL amp. You could easily run a micro-amp in that space for just a sub.
2. It fits nicely into the 4Runner since it was made for it, you will want to source the plastic panel in front of it that has a speaker grille as well, otherwise your options are to remove that panel and it will look kinda unfinished, or install your own speaker grille.
3. You'll want an 8" shallow mount sub. Depending on the sub, you may have to trim some of the inside of the box, but that can easily be done with some tin snips/dremel.
I have pics of my sub swap (in factory housing) in my build thread in my signature, a quick search will turn up lots of similar threads of people swapping theirs.
