Exclamation Steering Wheel Vibration

I am not new to the site but I finally registered! I took my '04 LTD into the dealership for the infamous steering wheel vibration, on Monday. I have had the vibs for at least 15, 000 miles (now at 69, 500 miles). I waited for the Dunslops to hit the wear bars (60, 000 miles) and replaced with Yoko Geolanders G051. Vibs were still there. I then had them Road Force balanced at the dealership. Smoother ride but vibs still remained. Tech rotated tires, vibs still there. I directed them to the Lexus fix, they printed out a copy of the thread, which coincided with the TSB from Lexus. I am currently waiting for the regional tech rep to evaluate. Dealership tech can duplicate vibs. The loaned me a Yaris S, big red nose and floppy shoes included. I have the Toyo Platinum Extended Warranty. Dealership said they will try to do the Lexus fix. I guess you can't see me holding my breath! I will keep everybody updated. Sorry for the long post.

What's the LEXUS TSB # for this fix? I think I'm having same issue in my '03 LTD and am in Russia, where Toyota dealerships are total shit. Taking my 4R to a local service station specializing in Toyota SUVs, might as well bring the printout of the TSB (I have membership to TundraSolutions)

did they use the lug adapter when they did the road force?
also known as the haweka?
you have to make sure they used it or else the wheel balance wont come out right trust me.. i had the same problem
took it to dealer for haweka balance... but sneaked round back and saw the guy using the regular center ,,, i told him to use the lug centric adapter.. done.. no more vibration
