Quick update here. I just wanted to close out this thread. I ended up taking it in to the dealer for a diagnostic test on the evap system. I figured I would be out the diagnostic fee but would know definitely what was wrong. Just like I suspected, it was coming from the top of the tank were I struggled to get the main vent hose off. The tech suspected the o-ring rolled or was torn. I could have walked away and paid the diagnostic fee but ended up letting them go forward. After further investigation, the tech said that one of the o-rings was torn.
It does sting a bit to pay so much money for a single $70 vent hose especially after doing a fuel sending unit myself for $160 which included a new retaining ring, gasket and fixing a broken tank strap bolt. My logic at the dealer was once I heard what was wrong, I was already in it for the diagnostic fee plus the cost of a hose (~$250) if I took it home and did it myself. At that point, I just figured I would spend another $200 or so to have it done.
That's how this P0441 and P0445 story ends. A bit embarrassing especially after doing a head gasket job myself. To be honest, I really didn't want to go back under the truck.
