Easiest way to tell imo is driving next to walls and slowly give it gas so it has a load on the engine, you'll hear it instantly.
05 v8 just rolled over 197k

My daughter just purchased one. It has 156k miles on it and just found out it may have an exhaust leak manifold issue or heat shield may need fixing. Wont know until this Friday when they get under it. We also found out that it has a wiring bypass for some common ac issue. So it results in lower air pressure to the vehicle occupants even on full blast. It also may need a starter? No issue yet but my daughter heard some weird noise And delay when starting thebkther day. It almost feels like driving a used vehicle off the lot and it malfunctioning right away. Just don’t know if we should fix basic stuff now then sell or if this is normal for year make and mileage. Anyhow my question is this a bad year or model for the 4Runner? It seemed to b taken care of but my Toyota experience is limited. My husband had a Tacoma we bought with 130k miles. Ran hard every day til 190k miles. Zero issue. He got in an accident. Only reason we don’t still have it. Only major thing was cv joints. Just did basic maintenance. Anyhow this was a 1.5hr distance purchase and just not sure if this is normal ware and tare. Help. It’s my daughters first vehicle. She used all of her money.

I know I've encountered the Heat Shield coming lose on 2 different occasions. The bolts and nuts holding them tight were rusted out so there was a constant knocking or metal hitting metal rattling, that was an easy fix. My faint ticking from the engine while being idle is something I'll need to keep an ear out for. I've had some leaking in the cv boot and my front brake calipers seized. I've owned it since 70K and that's what I've had to replace. From what I've researched my 08' should run pass 300K miles but obviously things will get worn and need replacing. But the expecting my engine, tranny, transfer case to be strong and long lasting.

Yeah, I had a dustshield rattle...I removed it and now it doesn't rattle anymore!
These engines also just like to tick. You can read up on it on here if you'd like, but it's not normally something that needs to be worried about. IIRC the V8 was the one that had exhaust manifold issues.

The biggest issues with these seems to be seizing brake calipers. Some early 4th gen V6's (pre-'05) had headgasket issues.
Mine's at 185k and the list of replaced parts (apart from things like brake pads, spark plugs, and tires) is: - Balljoints (I just replaced all front control arms for the sake of having everything new and to get adjustable uppers for my lift)
- FR wheel bearing
- FL RL brake calipers
- Radiator
- CV axles
- Shocks (I debate whether or not they were actually bad, cuz they weren't leaking and felt fine. But a shop failed them during inspection so I had no choice but to replace)
Not bad for a truck with that many miles. I'm sure most of those items wouldn't have failed yet, either, if I didn't beat the hell out of it offroad.

Assuming the other services were done at 90 and 180k miles, I'd do another timing belt job if I were you. That's not something you wanna let go longer than you should.

Yea, I hear ya,it's my 4th 2UZ. Did the air injection pump bybass from slyfox, but, I think it's like having kids.....after the 3rd timing service on "new-2-you" rigs seeing perfect belts with all teeth, no damage or cracks, you're like....meh.

Just bought mine, 2005 V6 4.0 Sport with 140k.
As my first real car, the mileage scared me a little, but everything checked out except for minor quirks that are bound to happen with a 14 year old vehicle.
