This time the brakes didn't work!
Since the last thread was hijacked by some people with nothing better to do than argue about who's car's headlights are better.
First it was the throttle body yesterday the brakes didn't work. 2015 4Runner with 8000 miles, wife pushed the brake peddle to the floor and nothing happened, let off and the second time fortunately the brakes locked before she rear ended the car in front of her, so close the guy got out to check to see if she was ok. Oh and then the car just died.
Anyone else have similar problems? Just a bad car or is Toyota's legandary reliability coming to an end. My wife is a life long Toyota driver and used to race in the Toyota Fun race in Venezuela in several different Machito's (wish we could get one here) and this is our second 4Runner together! I didn't think we bought a Jeep.

Huh.
Sorry about the accident but the above statement doesn't make sense. That's not how brakes fail, sorry.
User error is what I'm going with.

I can't imagine what would cause the brakes not to work like that.
I would not trust any dealership to diagnose something that critical. I would demand that Toyota (not the dealership) be very involved in your situation and get documentation on the claim and findings.
I'm not going to call your wife a liar..., but just be prepared for people not to believe her story.

Well that sucks. Sorry to hear what happened, man. It's one of those things that you wish it didn't happen at all and you start to loose trust in the vehicle or even the brand. Unfortunately, there may always be 1 in a 100 or 1 in a 1,000 vehicles that have a serious safety issue no matter how reliable the brand is.
Hopefully it's something simple they repair and that it never happens again. Keep us updated though.
BTW, you sure you have one floor mat in place and you verified that it's secured. J/K. Good luck with everything.

Being that the brake system is entirely mechanical/hydraulic when the electronics fail, I don't see how they could have been pressed to the floor the first time and the second time it worked, when the braking system has been working fine so far.
The only thing that I can think of would be a faulty master cylinder. But you'd have signs that it was failing long before it actually did.

I know the first couple of times of hard braking (and not so hard) I notice a distinct difference in the sense of urgency as far as feedback to brake pressure applied. I'm sure there's a name for this, but it sure took me a while to get used to it, and I had a few close calls.

I had that same break issue happen to me on a dirt bike once back in the day. Pull the break lever, nothing, do it again and it worked just time.
Hmm, I'm just as flabbergasted though. BC even with the dirt bike, it never happen again.

If you've ever done a brake job, you know that you need to pump the brakes afterward, or the pedal WILL go to the floor, and then become solid again.
To say that it's impossible for this to happen isn't correct. There's lots of things that could happen. Leaky master cylinder, some air in the system from a leak-- I'm sure a competent mechanic will find the problem.
But by all means, let the armchair engineering begin.

If you have a bad hub, it is possible for the pads to be "backed" away from the rotor on one wheel. The first brake pump puts the pad back where it should be, the second brake pump actually stops the car. It's not very common with todays brakes, but it has happened.
Edit: Or air in the system, as the previous poster mentioned.
