Door lock actuators. Have replaced two on my 2016. One under warranty (front driver) and one myself (front passenger). I did the $7 actuator motor replacement the dealer replaced the entire actuator.

Noise in front end, was the hood squeaking on the rubber stop. Dealer was an a-hole about it.
Idler pulley(s) making noise.
Steering wheel shake.

I voted no issue but I do have one. Incompetent grease jockeys that screw shit up for a simple oil change. I prefer to have it done but after reading 100s of horror stories I'll do it myself! In all fairness most brand dealer service are the same now. All thanks to corporate greed and a new generation of entitlement

The only issue I had was the needle bearing needing replaced after 8K miles, drove me nuts but it was an easy fix and the stealership knew about the TSB before I even showed up

I use my phone (Tapatalk) and can't see polls.
2017 SR5, 23,000 miles on it.
Zero issues, everything works properly. Absolutely no complaint.

If you can call it an "issue", air bag replacement.

2016 4runner : passenger pillar squeaking. Never could get it silenced.
2019 4runner: passenger pillar still squeaking but less prominent. Front suspension squeak, dealer replaced control arms.

In my 2017 SR5, I’ve had the needle bearing issue (at under 10K miles); the radio/navigation unit replaced (~35K miles); the TRD skid plate bolt issue (~20K)

2016 TE. I have had two issues. For some reason my parking break sticks when engaged. Dealer say the cable stretched and quoted me $500 to replace it. Extended warranty would not cover.
The second issue was a little more frustrating. After several misdiagnosis’ by the dealer, I learned that a wheel sensor connected to the speedometer went bad. $350 repair and the extended warranty did cover it.

Not really sure if it's an issue but after a drive I get out and smell a type of rubber/electrical smell. Like something burning.
At first I thought I didn't add enough gear oil when I changed out my diff fluid but I'm sure it wouldn't smell like rubber or electrical burn smell plus I added 3 quarts of Redline as per instructions.
I only smell it when I park it in the garage and assuming because I'm in an enclosed area I can smell it.
Unsolved mystery.

2018 ORP. Two issues:
1) Rear engine seal leak
2) Transmission leak occurred shortly after rear engine seal fix
I forget what the mileage was when they occurred, but it was probably around 20k-25k. Both issues were covered under warranty.
I know there's a thread somewhere on hear where other owners were experiencing the first issue. I forget the dealership's explanation for the second issue, but given the timing, part of me thinks it was a result of their work to fix the first issue.

2018 TRD Pro with 25k miles
1) The steering wheel is peeling
2) Brakes needed replacing at 20k miles (granted I do have armor, front and rear bumper, larger tires and use it offroad a good amount)
3) Intermittent bluetooth issues between phone and Entune (this can probably be fixed with software updates on both Toyota and Android/iOS)
