Did you go to each page, save to pdf, then combine them and create the TOC?
Or did you find a way to automate it?

Download older versions of Adobe apps
You can download it from here if you already bought an older version but lost the original install file.
If you don't already have Adobe Acrobat, you can get the Standalone version or DC (monthly subscription) here:

If you only plan on using it one time and want to save money, I would buy "Acrobat Standard DC" for just one month.
Under "Acrobat Standard DC", click "Buy now", "Monthly" and then fill in your payment method and buy it.
I would NOT choose the "Yearly Billed Monthly" subscription!
Even though it appears to be cheaper, there is likely an early termination fee that would cost you more money than the difference between the "Monthly" subscription amount and the amount you would pay monthly for the "Yearly" subscription.
- Be aware that even the "Monthly" subscription states "Cancel anytime, no fee", which means that you'll keep getting charged every month until you cancel it.
On the other hand, if you don't want to continuously pay a subscription and don't care about the latest features, then you can choose to buy a "Standalone" version:
Acrobat Standard 2020: $299
Acrobat Pro 2020: $449
