I did a little research last night. It appears the air injection pump only runs when the engine is cold. By injecting air into the exhaust stream it burns the unburnt fuel which not only reduces unburnt fuel emissions but it also warms up the Catalytic converter faster and again reduces emissions. It's final benefit is to help the Catalytic converter last longer by preventing that unburnt fuel from reaching it. Just wanted to clear that up, i hate misinformation in forums. So, it seems the air injection pump doesn't run in any other condition once the engine/cat is warmed up. Again, not 100% on this.

OP, any updates on root cause?

I took it to the original dealership I purchased it from today and had what they said was their best technician ride along with me. At first acceleration pulling out he said, "I hear it, and that is NOT normal."

He apologized for all the misinformation given to me and said his initial reaction is that does not have anything to do with the engine.

He agreed the sound is coming from behind the tachometer, but doesn't know what it is. He said he would do a build inspection, to make sure it isn't somethin obvious from quality control. If unsuccessful he will then start pulling things apart and investigating even more. If unsuccessful after that he is going to make a recording and get in touch with Toyota's service bulletin line. AND if that is unsuccessful they will be requesting a visit from a Toyota engineer.

Well, I was called and told the problem was solved! I went up there and test drove it to verify, and if has been fixed!!

The tech said it was the steering boot located above the pedals. He said every time the 5th cylinder would fire it would cause vibration to occur to the boot. To fix it they added some additional silicone lubricant to the boot. I am not very handy when it comes to anything other than Jeep Wrangler/Gladiator lifts, so don't shoot the messenger.

None the less, this has been a HUGE headache that hopefully is behind me.

They did create a service bulletin because they said this problem was a bear to diagnose.

Would have never guessed that root cause and thanks for the update.

I had a few annoying and hard to diagnose noises when my T4R was new. When dealer's technician was able to solve them, they became my instant heros and I am still grateful to those techs who persevered.

Glad the issue was solved. I too would have never guessed that would cause the issue.
What stumped me was the cadence of the noise didn’t change throughout an rpm increase.

But now, since you have been in touch with the lead tech at your dealership, you should keep requesting him to work on your T4R whenever it has to go in for service.

I need to look for this steering boot and see if it rattling is one of the many rattles and pops in my car. It’s so bad, I will actually feel guilty selling it. The next guy is going to be pissed.
