123,000Thousand miles
If I have 123000 miles on my 2011 4 Runner. When should I change or service transmission FLUID or Tranny.

If I were you, I would exchange the fluid now. I bought 3 gallons of MaxLife and performed the first fluid exchange at 114,000 miles almost a year ago. Transmission shifts great and no leaks. I did not replace the transmission screen.

um...now seems like a good time...or you can do it now.
but you might can put it off a little bit and wait to do it NOW
i did mine at 100K miles in January. I am at 135,000 now...drives like new
I made a full fluid swap on all fluids right around the 100,000 mile marker
trans, front and rear diffs, transfer case, radiator, oil change...Ive even swapped brake fluid last month at 130k.
everything ive seen and read says to swap that fluid between 90k - 110k

I do a "spill and fill" every 30k or so. Bought mine with 35k and did it at 40-ish and again around 70k. I'm nearly at 100k now and I have a gallon of valvoline ATF on my toolbox at work. I just put it up on the 4 post, drain the pan, refill it with a gallon, plug in my scan tool, wait for the ATF temp to get where it should be (I can never remember so I look it up every time), and pull the check plug until it just runs out as a trickle. Put it back in, done. It's time to do the diffs and t-case again on mine too. Just did the coolant, plugs, and a new belt last weekend.

You should change it now, if you plan to keep long term.
I just did my 3rd transmission fluid change at 100k intervals. I would recommend dropping the pan to clean it out and change the screen filter at 200k miles. I just did mine, and should have done so sooner.

123,000Thousand miles... WOW, that's like a world record! 123,000,000 miles!!
Agree with others, some time NOW would be great. Or you can go with the "if it ain't broke, don't fix it" and report back to let us know how long the trans goes without ever changing the fluid.
