With my current set up (E-rated 275/70R17s, slight lift), I'm averaging only around 15 on the highway.
It's all very dependent on wind and speed. A few months back I was driving across New Mexico with boxes on my rack and into a stiff headwind. I got a jaw-dropping 8 mpg for a stretch.

Was on my way back from Santa Fe to Houston recently.....for about 150 miles with the wind and downhill I got 21 MPG. Running 285/70/17 Toyo's. Real world MPG ave is around 17.
That's strange. I also have an AFE CAI, but your tire size should equal better mileage. However, if your KO2's are E load they are very heavy and "may" be a factor. I got rid of my E load KO2's for the C load Ridge Grapplers and wish I would of started with them.
I also have a Sprint Booster, which depending on how you drive, can improve overall mileage, but just by a little. Maybe that's the trick... but who knows.
Just picked up a Sprint booster, from what I read it seems it could help. I'm desperate!
2018 T4R Limited AWD
3500 miles - 17.8 MPG per the dash computer @ 6500' Elevation
4800 miles - after a road trip to Northern NM and AZ - 18.4 MPG per the onboard computer.
As usual, I reset the on board MPG meter at the gas station. Pulled out of the gas station on an empty road with a slight down hill. When I crossed the bridge the MPG meter read 27.2 mpg.
Alas, when I got home, it was back to it's usual 18.7 mpg.

lol, want to see it with some crazy numbers? Turn the key on, DON'T start the motor, then put in neutral and coast for a bit, that is if the gas station is set up where you can coast out of the pump area.
It’s really inconsistent. I’ve had several 4Runner rentals and I drove them like I stole it and have had up to 22 mpg. On my personal 4Runner I drive like Grandpa and I get 15.
SR5 with TRD Pro wheels and tires lift/leveling kit TRD intake and Pedal Commander on city +2
I average anywhere between 16-18 city-highway combined depending on how I drive.
