Wouldn't they constantly be in really low gears with the engine screaming?
It's bad enough with the 4.0 having to stick it in 3rd for half the drive to the tunnel.

Not in my experience. Modern DIT engines make most of their power down low and with a proper transmission you're hardly ever at high RPM unless you're really flooring it.

Edit - Also, having a turbo helps negate a lot of the altitude induced power loss.
