Now I am at a loss!
I have some people saying something has let loose ie cam sleeve bearings or lifters... maybe I should consider a new engine? I would like to get a diagnosis but am not sure what to expect.
Any doses of experience or wisdom welcomed...

You should post your question in the 3rd Gen 4runner section to get a better response.
Here are a few questions.
Does the motor start and run? If so take a video sound recording and post it.
loss of power etc. Is the check engine light on? Check the flex plate for cracks. When it cracks it makes a rattling noise that seems to come from the rear of the engine. There is a cover on bell housing that you'll have to remove to inspect the flex plate.
You can remove the valve covers and look if there is a loose shim.
