391k miles is insane, how many water pumps and rear axle seals have you been through?

My 4runner had 185,000 miles on it when trans cooler in the radiator hemorrhaged. Luckily no Pink Milkshake. I'd say ur transmission life is precariously tied to your radiator life.

I've had the same 99 Limited as you since 2002. I can tell you from experience, change the timing belt, radiator ( trans cooler problem ), rear axle seals ( I bet they're leaking ), water pump. I've had to replace these all multiple times due to failure... the timing belt never failed.
