You should replace the servo motor. You could try taking it apart and cleaning, but likely won't fix it. If it were me I would just replace the servo and move on. They can be tricky to sneak out (I can't remember exactly how tight it is on our trucks, but I do a few toyota/lexus a year at work...

Sounds like fuel delivery issue. Either through contamination in the tank or the pump failing.

Ok then those symptoms definitely indicate headgasket. Misfire on a cold soak is the symptom we always see. To verify we usually run them to operating temp, pull plug on suspect cylinder and then pressurize cooling system. As the engine cools you will find a pressure loss and coolant seeping...

It is one of two things. Either fuel contamination, in which case, just top it off and drive it and it should clear up or carbon build up on the throttle plate... in which case clean the MAF sensor and throughly clean the throttle plate. What are your short term fuel trims at in closed loop?

And FYI on our v8 engines banks 1 and 2 are reversed so cylinder 2 is the front cylinder (closest to radiator) on the passenger side

Reread your information you have a 4.7l I that you had the 4.0 v6. It's definitely a coil then.

Inspect the plug and swap coils with cylinder 4. If the misfire follows to cylinder 4 then you know you have a bad coil. If it stays on 2 then it's either the injector (not common on these engine) or the head gasket (most likely).

High input means open in the harness, ECM is seeing source voltage. Measure the resistance from the harness side connector to the ECM, likely an obvious broken wire under the truck.

Pull the MAF sensor and look up into it with a flashlight may have some debris in it from transport, spiders are a common culprit.
