Found the problem. When shifter is in D, the 1 input to ECM is getting 8v thus it believes the shifter is in 1 and never tries to upshift.

Levels are fine. Happens both cold or when at operating temp. When D is working and shifts into second it can shift normally in all gears.

I have a 97 with an intermittent problem. Sometimes when in "D" the transmission will not shift into 2nd regardless of vehicle speed/rpm. However when this happens if it is put in "2" it shifts normally from 1st to 2nd. I have checked the output at the ecm going to the shift solenoid...

One thing is to not wear synthetic fabrics, they really hold smells.
