Yes, pretty much once the temp is high enough to register on the thermometer the surging stops. That said, the power loss issues don't go away once the engine heats up.

I had this code right after I half-assed cleaning my MAF sensor. What I did was fully clean it and reset the ECU and it hasn't come back (that was months and a couple thousand kms ago). Not saying that it isn't the O2 sensor, by any means, but you may as well try and clean the MAF to see if that...

My 99 4runner has decent front springs (fatigue-wise) but are a little rusted. I'm going to replace the shock absorbers anyways, so is OK to re-use these? Otherwise I'd rather just replace the springs while I'm at it.
