Another vote to reboot. The kit you get from Toyota has everything you need, including two different kinds of grease for inner/outer. Once I had mine out and clean, the wear surfaces looked like they had 5,000 miles and not the actual 175,000.

My 2000 4x4 does the exact same routine, and I mean exactly what you described. I just shift into low and manually run up through the gears and by about 200 feet it's back to normal. Happens so rarely I just decided not to worry about it.

I think for light duty I'd just leave the system as-is and replace the radiator. It works well enough and you'll be set for another 10+ years.

My "OEM" radiator came in a Lexus box and has a Denso sticker on the top. Looks every bit the quality part as the original, and yes, buy from your local Toyota parts counter guys. Unbox it on the spot and check for damage.
