almost same situation. 94 4runner 3vz-fe. i have been running o% ethanol and having no codes. put 10%ethanol (Standard) and the code comes up. it goes away almost as soon as I fill back up with 0%. Im thinking that it's just old and will have problems.

same I had that problem. I usually come to a COMPLETE stop then shift into reverse as i am letting the clutch out a little. this works most of the time. once i get it in i slam the clutch pedal down. it works but should be checked out.
