If you do it right, it is the "post-cooler" as it cools the fluid after leaving the radiator cooler. There'd be minimal benefit in cooling the fluid before entering the radiator cooler, and much greater impact if you cooled it after (which is immediately before it returns to the transmission).

I had Rok Blokz on my 2nd gen Tacoma which had side exit exhaust (standard). The Tacoma was listed as a supported application for them and I never had any issues. Unless the distance between the exhaust and flap is shorter for the 4Runner I wouldn't think you'd have a problem.
