I've had the same symptoms in my '08 V6 4Runner. Persistent p0174 and p2197 codes after replacing the MAF, AF/O2 sensors, spark plugs, gas cap, and most recently replaced fuel injectors on bank 2. After the fuel injector swap it ran really well for about 25 miles, then threw the codes again...
