I will chime in and say with 285 BFG KO2's in C range on 8.5 inch wheels, I am averaging a combined 16.4 miles per gallon with a 3" lift and a C4 Lo Pro Bumper.
You went from P rated tire 265's (if that's what you meant by stock) to E rated 285's... that's probably 2 miles per gallon. Lift making you less aerodynamic lets go with 1 mpg, and the weight of the rack is 1 mpg. Just throwing a few things out there!
Can't find the other 1.2 mpg, but that gets it in the ball park. Biggest culprit being tires in your scenario. I'm guessing you have an app or are doing the math on your MPG's rather than trusting the computer on your 4runner.
