I assume the service history printout from a dealer is sufficient? But, for instance, if she didn't have every single oil change done at a Toyota dealer, will the transfer be denied? Also what exactly is a "roadworthy inspection report"? This car is like new with nothing wrong at all but there are no details on who is to do this report, what form it is to be on, etc. All of this sounds as though it's just details to deny your transfer saying you didn't submit the proper paperwork...
Comments or suggestions or past experience with this?

This inspection can be done at the dealer. I don't know what part of the country you are in, but I know here in Cali THE PEP BOYS stores also provide this service. Is she hasn't done any maintenance at the dealer, hopefully she saved the receipts. I would call Toyota on this. Make sure you have the VIN of the car you are looking to buy. If she didn't save the oil change receipts, or didn't perform the oil changes, that could be an issue. Just because it's a looker doesn't mean she changed the oil.
