2002 4runner sr5 2wd--transmission pop

Ok, so i have a 2002 4RUNNER SR5 2WD and everytime i shift from reverse to drive or vise versa, it just pops, almost like its going to snap or something. Keep in mind it has over 230+ thousand miles on it, its never given us any trouble, we've changed everything as needed, oil, belt, and so on and so forth.. Is anyone else having/or had this problem? If so what were some of the reasons for it?
One other concern, vibrating brakes? Whats the deal with that? ANY AND ALL REPLIES ARE APPRECIATED!!!

Are you sure it's not your driveshaft? 2wd 4runners generally don't have zerk fittings on the DS so you can grease it but i would check and see if yours does. Any squeaks or vibrations when driving?
