4Runner Shudders When Accelerating From A Stop

I have a 1997 4Runner SR5 2wd.
It shudders quickly then becomes smooth when I accelerate from a stop, especially when I am towing. It does not shudder at any speed greater than 5mph. It dosent do it all the time, only sometimes. It has been going on for about a 1-2 years, I really dont notice unless im towing, and it dosent seem to get worse.
In the last 20k, while the shuddering has been going on, I have done all plugs and wires, both U-joints, rear differential fluid change, and transmission fluid is still red.
I drive it hard, I do all the work myself, and I make sure its done right, so Im hoping this is nothing too big so I can fix it myself! And ideas what my problem could be?

Transmission mount? Have you replaced any of the motor mounts?
 
No I have not, are they hard and/or expensive to replace?
And if I dont fix that, wouldnt it get worse with time?
If this is the case, am I hurting anything by not fixing it?

Ive changed all my mounts, and it was a pain in the ass. Unless you have the proper tools to lift the engine out, i guess it would be damn near impossible. The mounts are cheap, i got all mine from a buddy down at Toyota For 100 Bucks, I think they are all around 40 to 50 bucks a piece paying full price and buying OEM.

Is your suspension stock?

I had this issue with my 2006 Tacoma, it was due to the drive line angle coming out of the rear differential after I put a lift on it.
