Stalling at idle, hestitation, bad transmission??

My 4runner use to stall at red lights and I thought that I had fixed the problem a long time ago. Now the symptoms seem to be coming back, however, something seems different about the car now. It has a slight hesitstion shifting between gears.

The last time that my 4runner would stall at red lights, during idle, and in gear, I changed out the following:

IACV
TPS
back o2 sensor
Cleaned the throttle body
Engine coolant temperature sensor
Spark plugs (wires are new, coils are not)
PCV valve
Fuel filter changed at around 200k.
MAF sensor

My truck was symptom free for 12k. It now has 230k on he odo. Orginial engine and transmission.

Any suggestions?

If no CEL present, double check for a pending code.

If nothing, I'd proceed with a 'standard tune-up' (wires/plugs/intake cleaning with seafoam, ect.).

I can double check the spark plugs and things, but those plugs have less than 20k on them. What does seafoam do? You just pop it in the gas tank right?

Get a can of sensor safe throttle body cleaner and with the engine idling shoot the bypass port the IAC gets its air from a few times and try and flush any dirt out.
