Thumping clonking noise

When I first crank my 4Runner it makes this clonking noise and I can feel it like its trying to shift. Or something. It also does it when I brake for a redlight and then go to take off again... It's like, we'll I don't really know how to explain it.
It shifts really smooth and runs excellent but it does have 199k miles on it. Anybody have any ideas?

Why would it do it when I crank up. The drive shaft isn't moving. It's before I put it in drive.
