Upgrade 2018 to Support CarPlay?
What are my options for Apple CarPlay on my 2018 T4R?
Will the 2019 T4Rs support Apple CarPlay? If so, could I just swap out a 2019 HU on my 2018?

According to Toyota's website it doesn't.

if you want CarPlay you're best investing in a legit aftermarket HU, it will be much cheaper to get into these than swapping vehicles, and if you spend the right amount (well less than $2k unless you like fancy bidets and such), you'll get far more features than you currently have with entune

Pioneer AVH4200NEX, going for 400 bucks on Amazon right now. Add about 100 bucks on top of that for the cables and adapters for steering wheel controls, etc. Do the work yourself and for only 500 dollars you've got both Apple CarPlay and Android Auto.
I've had that head unit for 2 years now and I'd buy it again. A few very minor complaints but I still love it.

Aftermarket with carplay/android auto is the way to go. I as almost ready to trade in the 4runner for something with android auto because I really do need maps on a larger screen on the dash without resorting to ram mounts or doing something that I find unacceptable to a new vehicle. That said, going with an aftermarket stereo (Kenwood DMX905S) was a great choice and made me 100% content with the 4runner.
But, you'll struggle to decide what to do regarding the dash kit. Scosche is far better quality than Metra (though Scosche is still kind of...meh), but only Metra currently makes one that's painted black. Some people here have painted their Scosche dash kit, which I still plan to do despite having just installed the black Metra version.

I haven't done the conversion in this vehicle, but it's definitely worth the effort to have it professionally painted.

curious, because I don't see it in the description, but do all of these have the backup camera integrated into them?

Most of the modern HU will accommodate, but that's why I use crutchfield, because I can't sort that stuff out myself. Your biggest risk is whether the HU will support the steering wheel controls or the JBL installation, if you have these. NAV and camera are pretty standard with top end double-DIN units now.

Building on Toffees' comment: If you get a radio that's idatalink maestro rr compatible, it solves this in most cases, while also adding in several features like the TPS monitor and several other temperature gauges. My unit, that I listed above, also connects wirelessly to my phone for AA and has a dedicated camera button so that I can switch between the HU display and my different camera inputs at my will, while also retaining the factory back up camera usage (ie: when I reverse, the camera switches to the screen).
Its all about just finding the HU that suits your needs for the price you're willing to pay. Crutchfield is incredibly useful, but it should be noted that if you have a 2014+ Trail or TRD Offroad or TRD Pro, you'll need to either have the dash plastic that comes with the kit professionally painted or shell out $80 for the new charcoal metra kit that just came out. While that is cheaper than the Scoshe alternative, metra dash kits have been dropped by Crutchfield due to customer complaints on quality.

New Head Unit
I just got my 4 runner, first thing I did was sound system
I put this in. It has wireless carplay (you don't need to plug your phone in). I have had it for 2 weeks, so far it's been fantastic. Nav looks good through maps and back up camera with guild lines looks perfect. Steering wheel commands all retained.
