Thanks to everyone. After I aligned the cam and put the timing belt on it felt like a whole new truck! I still am amazed that it was running at all, the cam was pretty far off.

Yes the fuel filter was replaced about a year ago (4k ago). I think I have found the problem. the cam is off, i found the timing belt loose, the idler pulley has a nearly frozen bearing, and the tensioner is shot....I am suprised it was still running! I'll post back later when i get it resolved.
