Hmm i aint had to much experiance with 4runners but if its my own personal opinion sounds more like the gearbox... my sisters old car did a gearbox and it is doing exatcly what you explain....
New update... smokes stopped so figured was oil on the exhaust side and intake due to being on big angle... driving is half ok does slip sometimes but reverse wont work it only jerks
