256k on '96 If you give a regular maintenance it should last to 300-350k not a problem. My has 256k and still run strong.

The rotor is Meyle (German made, RockAuto AZ Carry them) and pad is OEM

Bad Injectors is Good Possibility I'm very much agreed with T4Rscott. However, depend when is the last time you replace the plug wire, coil or spark plug, it might be the easiest thing to look into first. My '96 4runner has the same symptom #1 cylinder misfire and turn out to be a bad injector...

I have a 96 Limited and the remote is black…. the green button is referring to is on the side of the unit where you can reset your remote. I also have a 98 Limited the remote is corrected grey.
