Contact info for extending Toyotacare plus

I'm thinking of extending my Toyotacare plus. The dealer wants 369.00$ which seems high. I've seen $299.00, $199.00 and even $99.00 to go to 45,000 miles. Does anyone have the contact info for the dealerships (i have not been able to find this in the threads). Thx

You're getting 2 oil changes and 5 rotations. Oil changes are approx. $50 - rotations at a local shop are $15-20. Seems like $200 is a break even if you were to pay for it at the time of service.
