I finally got around to finding and purchasing a dome light for my 4runner, and after installation, I was reminded that it was sporadic in the first place, and now with the LED it is just dim and crappy. Removed the dome light assembly, verified I was getting 12V out the plug, sanded the contacts, reinstall. The door switch works great! Nice and bright. However the 'on' position is either dim and flickers, or it just doesn't work. I had it set to 'on' when I put the assembly back in with the light, and it was bright until I put the rear screws back in. Now I cannot get it to light properly even without the screws.
Maybe RAD4Runner can depart some wisdom here!

For future reference since I had a hard time finding info, this bulb works great if you bend the contact tabs just a smidge:
