Low power when cold

Got an issue I can't figure out. My 98 has been running pretty sluggish when cold. For the first 5 minutes or so, until the temp guage comes up to the 1/4 mark, the truck will run boggy and will not pull past 3500 RPM even with the pedal pushed to the floor. It also has no power at all. The transmission does not seem to be slipping. Once the temp guage comes up a little more, it gets crisp and will pull to redline. I have cleaned the MAF, IAC, and throttle body, replaced plugs, wires. All belts are in good shape. I am at a loss. Also, it sounds like I have an exhaust leak somewhere under the right floor pan until it warms up. Then I don't hear it. I have tried to locate it and I just have no idea where it is coming from. I can't hear it unless I am driving and accelerating.

It's a feature built into our runners. Your truck will not shift out of 2nd until it has warmed up to a certain point then you will be able to shift properly.
