What a horrible commentary on the state of the auto industry, when prospective buyers are scared of buying a 3 year old vehicle.

Generally speaking Lineman is right on this. 65K on a 4Runner SHOULD be insignificant, but it all depends on how the previous owner(s) treated it. You can't just make a blanket statement that it's safe to buy a used vehicle. It all depends on how well the truck was taken care of. It is totally possible that a truck well maintained with 165K miles on it could be a better buy and serve you better in the long run (and be more reliable) than one with only 65K that has not been taken care of.
I would definitely check with Carfax as Lineman suggested. Keep in mind that a Carfax report is only as good as as the information reported to it. Equally important is ask the owner for all maintenance and (if any) repair records too. If they can show you everything documented in writing going back to when the truck was new, that's a very good sign it was well taken care of AND it proves they had each service done and when.
If they can't show all the records, you just don't know......the very fist oil change could have been at 30K miles. Also, during the very critical first 1000 miles, it's super important that the truck was driven very very gently, like a grandmother would drive it to properly break in the engine. This will be difficult to find out, but if a teen driver for example, had it when new, I'd be careful.
Again, as Lineman said, get a good mechanic (chosen by you, not the seller) to look at it and that should tell you volumes about how well it has (or has not) been maintained.
You can also tell a whole lot about how well the truck has been maintained simply by looking at it. Does that paint still look good? How do the seats and carpets and floor mats look? Are the tires wearing evenly, or does it look like they have never been rotated? Chances are that if the seller has not taken care of keeping the paint and interior in good shape, he definitely has not bothered to take care of the engine and drive train either.

Bought my certified 04 limited a few years ago with 79K on it. If it is certified you probably get the 1 year warranty. Just take the time over the first year to review all the posts on this site for potential problems and make use of the warranty.
I had my timing belt changed and they covered the water pump under the warranty. I had the brake fluid flushed and they fixed two CV boots under the warranty. I didn't know about the catalytic TSB that replaced the cats that had a rotten egg smell and crossed over the 80K limit before I figured it out.
The only other work I have completed on my truck is just wear items. Brakes. Nothing else and I now have 114K on the truck.
So go ahead and get it. Just be ruthless about looking for issues that are covered by the certified warranty and get them all fixed in the first 12 months you own the truck. One that comes to mind is to go ahead and get the timing belt changed right before your twelve months is up even if you are short of the 90k recommended interval. That way, if the water pump is leaking a bit they can replace it under warranty.
No need to pay if you don't have to.

I thought I was about to read a typo when I read that someone is worried about 65k miles! I just bought my '06 SR5 V8 2 weeks ago with 110k miles. Within the first two weeks I had the timing belt and water pump service done, so I have nothing to worry about. It runs like new and does not feel worn out in any way. 65k is a little above average for a 4 year old truck but as long as it has been maintained (and it probably has highway miles anyway), then you have nothing to worry about. Even if the vehicle only has 10k miles on it...you should get it professionally inspected...and look for maintenance records. And don't be fooled by a clean Carfax report. Just because there's nothing on a Carfax report doesn't mean it's clean.
If a good inspection and good maintenance records...then 65k miles is NOTHING. I have over 210k miles and only minor problems (ODD and rotors). I LOVE the way the truck drives and handles.
